Overview
This page contains links to external sources of public health data available at small geographies. At the bottom of this page we have purpose built health and wellbeing profiles for Primary Care Networks (PCNs) which have been designed by the Public Health team at Somerset Council. These provide insight into how health and wellbeing varies in different areas of the county.
Somerset Intelligence
The Somerset Intelligence website hosts local Electoral Ward and Local Community Network demographic profiles as well as a host of other information and resources including summaries of the 2021 Census.
Fingertips Health Profiles
Fingertips health profiles are produced and published by the Office of Health Improvement and Disparities (OHID). Fingertips is a large public health data collection. Data is organised into themed profiles. Most profiles will have data only at a Somerset Council level based on residents. But it allows benchmarking with England and Regional averages as well as other local authority areas. Local Health
Local health is a collection of health indicators presented for small geographical areas. These indicators are for local authority public health analysts so they can assess and target inequalities in specific areas. Local health enables comparisons between small geographic areas to a larger one, for a single point in time, and is not designed for time trend analysis.
The Local Health tool allows you to pick your own areas based on small geographies (electoral wards and MSOAs) to build a health and wellbeing profile for the population resident in those areas. The Local Health Tool can be accessed here.

National General Practice Profiles
If you want to look at health and wellbeing data at a GP Practice level this can be done by viewing the fingertips National General Practice Profiles (NGPP). These combine mostly Quality and Outcomes Framework (QOF) and GP Patient Survey data and can be accessed by following these links:

Cardiovascular Disease Prevention Audit (CVDPREVENT)
The Cardiovascular Disease Prevention Audit (CVDPREVENT) is a national primary care audit that automatically extracts routinely held GP data. This tool provides open access to the data, with clear, actionable insights for those tasked with improving cardiovascular health in England. This tool allows you to access data at GP Practice and Integrated Care System (ICS/Somerset) level.
The tool can be accessed on the Cardiovascular Disease Prevention Audit website. A demonstration video is also available on this website which shows you how to use the tool as well as additional information.
Somerset Primary Care Network Health and Wellbeing Profiles
The Somerset Primary Care Network (PCN) Health and Wellbeing Profiles are designed to give an overview of the populations health and wellbeing, local service activity and community assets to help identify areas for exploration and prioritisation. A profile has been created for each of Somerset’s 13 PCN areas to support the locality working model adopted by the Public Health Team at Somerset Council.
