Referrals Forms
Health professionals can refer people into the service by using our online referral form here or you can download our referral form here which can be sent to us by email: Smokefreelife@somerset.gov.uk
Alternatively, if you would prefer to make a referral by phone, please contact us on 01823 356222.
Midwives can use Online M2B referral form for midwives to refer patients into our service.
Patients can also self-refer into our service either by using our self-referral form here or by phone on 01823 356222.
Smokefree Families
Smokefree Families is a service for pre-conception, pregnancy and any significant others who want to quit to support someone in pregnancy. It is also for any household with children under 5 years old. Support is offered in the home and by phone.
The service provides information on:

- What is on a cigarette and what are the health harms
- Benefits of quitting
- Smoking in pregnancy
- Smoking and the family
- Secondhand smoke
- Raising the subject
- Very brief advice
- Ambivalence and rolling with resistance
- Referral pathways
- What the Smokefree Families Service offers
- Resources and further training
